The condolence entry form allows users to leave a condolence message in the guest book. The one shown to the right uses the long version of the name and address entry. The short version, shown on the guest book entry may also be used for the condolence form.

The web site responds with a personalized thank you after a person submits his/her condolence, providing confirmation to the user that the entry was recorded. An example of a response page is shown below. You may customize the language used in the response.
